A total HERTSMI-2 score of 0–10 is considered generally safe for most people. Not entirely. A score below 11 is reassuring, but as Dr. Diane Mueller notes, patients with CIRS or HLA mold gene susceptibility may still react to relatively low mold levels. Additionally, the HERTSMI-2 only tests the five most common illness-causing species — other mold species not covered by the test may still contribute to symptoms. If your score is low but you continue to experience unexplained chronic illness, a full clinical evaluation is the appropriate next step.
Each species is assigned points based on its spore concentration (E/mg) using thresholds that reflect each mold's relative toxicity. Aspergillus Penicilloides and Aspergillus Versicolor score 10 points above 500 E/mg, down to 2 points at 1–9 E/mg. Chaetomium Globosum and Stachybotrys Chartarum use lower thresholds (10 points above 125 E/mg) reflecting their higher potency at lower concentrations. Wallemia Sebi requires much higher concentrations to score — 10 points only above 2,500 E/mg. The five individual scores are summed to produce your total HERTSMI-2 score.
